As the parent/legal guardian, I give permission for my child's data contained on this form to be stored by The Church of Jesus Christ of Latter-day Saints in Nigeria to be used by Church leaders and nutrition program workers authorized by the Church to assess my child's nutrition and health status and compile comparative nutrition program results for as long as necessary for the fulfillment of the objectives of the nutrition program of the Church. I also give the Church permission to use my child's photos taken during screening for the same purposes and to educate others about the program. I also consent to sharing my child's data with the concerned local government and health authorities to properly treat my child and address other nutrition and health concerns of my child.
