Child Nutrition Screening Program
Parental Notice and Consent

As the parent/legal guardian, I give permission for my child to participate in the Child Nutrition Screening Program, which is sponsored by The Church of Jesus Christ of Latter-day Saints (“Church”) and offered or arranged by The Church of Jesus Christ of Latter-day Saints (“Association”). As part of the screening, I understand that various types of information will be collected, including information that identifies my child, including name, birthdate, gender, and various physical measurements (height, weight, and arm circumference), along with any other information I enter on the screening form (“my child’s data”).

I give permission for my child’s data to be collected by the Association, Association employees, volunteers, or personnel, and other nutrition program workers authorized by the Association (collectively the “Nutrition Program Workers”), and for my child’s data to be used by the Nutrition Program Workers for the purpose of assessing my child’s nutrition and health status and compiling comparative nutrition program results. The screening process may require that my child’s data be entered into a form that will temporarily be stored in cloud storage. I give permission to the Nutrition Program Workers to enter my child’s data into such forms for the purpose of completing the assessment.

I understand that the result of the screening process is not medical advice. None of the individuals conducting the screening have been authorized to practice medicine. If the screening tool detects any issues of concern, I should take my child to a medical professional to discuss the results of the assessment.  

I understand that some of the information included in my child’s data, after it has been anonymized (so that it no longer identifies my child), may be shared with the World Health Organization (“WHO”), non-governmental organizations, or local governmental agencies for analytical purposes. Anonymized data may also be shared with the Church to give Church leaders a better understanding of the health and nutritional needs of the local population generally. Except as described in this notice (including the uses listed below), information identifying my child will not be shared.

I can decide whether information identifying my child can be retained after the screening event is concluded. If I do not consent to the retention of my child’s data, all information that identifies my child will be deleted at the end of the event as further explained below. I give permission for the Association to retain my child’s data after the screening event has taken place information identifying my child will only be kept until my child is five years old, after which it will be deleted.

I give consent to the Association to keep all my child’s data on file for the purposes listed. I understand that this consent is not required for my child to participate in the screening. If I do not consent to the retention of all my child’s data, I will still receive the results of my child’s assessment, but my child’s personally identifiable information will be removed at the end of the screening event.

I give the following consents regarding the sharing of my child’s data, including information that identifies my child:

1.       I give consent for the Association to share my child’s information and assessment results with both local Church leaders and program administrators, including sharing such information with the ward or branch council of the local congregation where I live, in order for them to counsel together with me and develop action plans for my child and my family. Data specifically identifying my child will be shared with Church leaders of the congregation where I live and will not be shared with leadership of other Church congregations.  Such data may also be shared for review by Church program administrators to help assess and improve the program.

2.       I give the Association permission to use photographic images of my child taken during the screening event for educational and identification purposes.  

3.       I consent to the transfer of my child’s data to locations outside of Zambia, including the United States, and to the storage of my child’s data in a cloud location.

4.       The Church upholds strict standards regarding the privacy of Church members as explained in the Global Privacy Notice of the Church (“GPN”). I understand that my personal data and my child’s information will be treated in accordance with the GPN, unless otherwise described above. If I am not a member of the Church, references in the GPN to joining the Church, membership records, religious ordinances, missionary service, or volunteer or leadership positions do not apply to me. If I live in a jurisdiction that requires consent in order to process my and my child’s personal information, I hereby consent to the processing described in this notice and as described in the GPN.
