Share My Health, Inc. (“ShareMy.Health”) gives you control of your health information (“Health Information”) and what you choose to share with those you trust.
Any health information you enter into the ShareMy.Health Platform is strictly private to you. Your health information will only be accessed with your explicit approval as detailed below. You can choose to enter your health information, including:
Bluetooth and Local Network access in order to securely synchronize authorized data with other devices running ShareMy.Health.
Files and images. Our mobile and web apps allow you to access the camera, files, and images on your device so you can easily upload copies of your records, including vaccinations and test results. You can also use the camera to scan applicable barcodes. Any files and images you upload have the same robust protections for access, collection, use, and sharing as all your other health information.
COVID-19 health information, such as vaccinations, test results, and self-assessments.
Wellbeing information, including physical health, mental health, notes, appointments, goals, and measurements.
Care collaboration information, including contact information for friends and family, messages to other people, and invitations.
Other health information that you in your sole discretion choose to enter.
By clicking "Accept" below, you agree:
1. to allow ShareMy.Health to securely store the Health Information you choose to upload, whether directly to ShareMy.Health, through its clinical and technology partners, or through other organizations you explicitly approve of through the ShareMy.Health software, and
2. to securely share, only when you explicitly approve, specific Health Information with people and organizations of your choice through the ShareMy.Health software, and
3. that you have reviewed and agree to the ShareMy.Health Privacy Policy and Terms of Service.
