You agree that Share My Health, Inc. ("ShareMy.Health", "we", "us") may provide you with Authorizations, Notices and agreements in electronic format. For example, your consent to receive electronic Authorizations and Notices includes, among other things, any authorization that we or our covered entity affiliates are required to provide or obtain from you under applicable privacy laws, including but not limited to General Data Protection Regulation (GDPR), Health Insurance Portability and Accountability Act (HIPAA), Personal Information Protection and Electronic Documents Act (PIPEDA), Canada’s Anti-Spam Legislation (CASL), and California Consumer Privacy Act (CCPA) (“Privacy Laws”).
All Authorizations and Notices provided to you in electronic form may be provided by
1. access to a website designated for such purpose,
2. access to a mobile application, and/or
3. by email.
"Authorizations and Notices" means
1. any authorizations required by Privacy Laws that we may request of or deliver to you from time to time during your use of the Services as well as
2. any and all notices, disclosures, and other information that we may be required by law to provide to or obtain from you in writing.
All Authorizations and Notices in electronic form from us to you will be considered "in writing".
You should print or download for your records a copy of this Consent and any other Authorizations and Notices important to you.
